 Agreement and Release of Liability

Life Centers Global Inc., DBA Bless Buncombe, and our partners appreciate your willingness to partner with and participate in God's work in our community. We have established these guidelines to help maintain unity and team spirit. Please carefully read this Agreement and Release, and then, if you agree with its terms, sign below.

Volunteer Acceptance

In consideration of being accepted as a Volunteer to serve our community either through Bless Buncombe or through one of our community organizations, you agree as follows: 

(1) You will present yourself within the scope of your approved Skills and Capacity for serving on one of the teams.  Example: Do not Represent yourself as a Professional Counselor, Psychologist, or Doctor unless you are. (2) You are not an employee of Life Centers Global or Bless Buncombe (3) All of your assigned responsibilities would be done unto the Glory of God and to bless our community.  If you have a conflict during your service, all reasonable Biblical efforts will be made to resolve the situation peaceably.  Bless Buncombe or the community organization may reserve the right to end this Volunteer Service relationship with or without cause.

Hold Harmless Agreement

As a volunteer, you discharge and release Life Centers Global, Bless Buncombe, our community partners, and our respective officers, directors, and employees from any claims, demands, and liabilities for property damage, personal injury, and death arising from your volunteer activities or participation in this community Initiative. 

You consent to and release any form of media exposure, including video and photographs of you, to be used as deemed necessary by Bless Buncombe or our community partners. You also covenant not to sue or to commence any legal action, complaint, or charge against Life Centers Global (Bless Buncombe), its officers, directors, partnering organizations, and employees regarding any matter covered by this agreement and release.

EMERGENCY HEALTHCARE AUTHORIZATION

In the event of injury or illness, whether actual or suspected, you authorize and permit to take you or arrange for an ambulance or other emergency transportation service to a doctor or hospital for medical diagnosis or treatment, including but not limited to emergency surgery or medication. You assume responsibility for all related fees and expenses incurred at that place.

By signing below, you acknowledge that you have carefully read this Agreement and Release of Liability and agree with its terms as binding for you, your heirs, legal representatives, successors, and assigns. If you are under 18, you must also obtain the consent, agreement, and release of a parent or guardian who is legally responsible for you, as reflected below.

CONFIDENTIALITY STATEMENT

This statement emphasizes that access to confidential information regarding a person's health record is limited and governed by federal and state laws. Confidential that motion includes the person's name, social security number, address, and medical, social, and financial data. Data collection by interview or review of documents must be in a setting that protects your privacy.

Bless Buncombe facilitates connections between genuine churches,  nonprofits, and businesses to uplift and serve the community, fostering hope and renewal throughout Buncombe County and beyond.

We aim to engage and empower local organizations and businesses to serve their communities collectively, driving meaningful change and fostering collaboration for a brighter, more hopeful tomorrow.

We will only share your information with partnering organizations that need your skills and volunteer assistance.
